This is a rare and very unique one-of-a-kind piece of memorabilia from arguably the greatest coach in professional sports history, the Zen Master, Phil Jackson.
This a seven page handwritten in black ballpoint pen questionnaire of Phil writing about everything from his transition as a player to coach and an in-depth look into his thinking, philosophies and journey to becoming the most legendary and championship winning coach in NBA history.
This questionnaire was done for a feature called “Enter The Coaches Mind” inside of the National Basketball Retired Players Association’s “Replay” magazine back in Spring of 2004. Please reference the handwritten Lakers note card from Phil Jackson to then current executive director of the National Basketball Retired Players Association, Mel Davis.
